As the end of the school year quickly approaches, we in the health room are already planning for the 2008-09 school year. As part of this preparation, we want parents to know what paperwork is required to be completed and returned by June 1, 2008. No student will be permitted to return to classes unless all the required paperwork has been submitted.
Emergency Information Form* is due by June 1, 2008
( a separate form must be completed for each child )
This form is essential since it provides all current contact information in the event of illness, injury or other emergencies such as an earthquake. Contact information should be maintained throughout the school year with updated telephone numbers for home, work, and current names of childcare providers etc. This Emergency Information form also provides information about any medical issues that may impact your child’s day such as allergies, asthma, headaches, migraines, nosebleeds, seizures, etc. By knowing this information, we are better prepared to care for your child’s needs. The school nurse will contact parents in August for those students that need to have a medical care plan developed.
This form also requests specific information on any on-going daily medications, emergency medications such as EpiPens for severe allergic reactions and as needed medications such as inhalers, Benadryl, Tylenol, etc. that the student may require. Based on this information, parents may be requested to provide medications to have available at school for the designated circumstances or for disaster preparedness purposes.

Authorization for Administration of Oral Medications Form*:

Parents who are either required by the school or those who wish to have medications available at school must complete an Authorization For Administration of Oral Medication form. This form is completed and signed by both the parent and child’s physician. The form is required in order for us to give any medications at school. This authorization is required for both prescription medications and/or over the counter medications.. Oftentimes students who have a headache or allergies come to the health room and ask for Tylenol or Benadryl. We are not permitted to dispense any medication unless we have an Authorization for the Administration of Oral Medication form that has been completed and signed by both a parent and the child’s physician and the parent has provided the medication in the original containers to the health room.
Only parents who are either required by the school or those who wish to have medications available should return this form. Forms should be submitted by August 1, 2008. Parent may bring medications to the school after August 18; however, ALL Medications must be in NO LATER than AUGUST 29th. If no medications are needed at school, then this form should not be returned.

Immunizations Card*:

All students of Sacred Heart School must have a completed and compliant Washington State Certificate of Immunization Status (CIS) card on file. Please review your child's records and ensure that your child is up to date on all required immunizations. It is important that children are fully protected against these dangerous childhood diseases. All student CIS cards are kept in the health room and should be updated as needed. (See the following link for information regarding the new requirements for Varicella(Chicken Pox).
Parents may claim an exemption to one or all immunizations by signing the appropriate box on the reverse of his/her CIS card. However, if an outbreak occurs at Sacred Heart School, your child would be excluded from class for the period of communicability for his/her protection. All CIS records must be signed and completed by the parent or legal guardian.
